Congratulations in taking this step to look after yourself in the best way. I hope this is a positive and transformative experience for you.
Before we begin, there are some things you need to complete to help make the experience run smoothly and also to allow you to get the most out of it. These forms are below. Please have these completed at least 48 hours before our session.
There are 2 essential forms to be completed before we meet, these are the Client Agreement Form and The Welcome Pack. They are all on this page.
1. Client Agreement Form – this gives clarity as to the admin processes and scope of practice. Click here to complete the client agreement form or use the embedded version further down this page.
2. Welcome pack – this questionnaire will help get you in the coaching mindset and give me insights to get the most out of our sessions. This questionnaire takes about 10 minutes to complete, so you can save this page and come back to fill it out when you have a bit of head space for it.
